Issue:

An Ex-Employee/Veteran requested a copy of who has accessed his electronic records. He stated a person's name and felt they were accessing his records inappropriately. The list was run and this person's name is on the list of accessing his record. The Privacy Officer (PO) is investigating the reasons why this employee accessed his records. Update: 05/10/13:The ex-empployee/Veteran will be sent a letter offering credit protection services.06/10/13:This was determined to be HITECH reportable by VHA Privacy Office.

Outcome:

The supervisor has contacted HR and is awaiting what steps to take next on this employee based on her inappropriate access into this former employee's record. Even though the employee said that this ex-employee asked her to look into his record - as a friend she did. The Supervisor worked with HR and the appropriate action will be taken on this employee because they inappropriately accessed this record. The appropriate action was taken
