Issue:
Veteran A asked for an echocardiogram to be released to him to take to his external physician. When he was at his providers office it was discovered by the physician that Veteran B's had been uploaded into his record. This was the name, full SSN, date of birth, and echo results of Veteran B. Veteran A immediately notified ROI supervisor of mistake. Update: 06/28/12:Veteran B will be sent a letter offering credit protection services.
