Issue:

Wife of deceased Veteran A requested all 7 volumes of veterans medical records. Upon receipt of the first 6 months of medical records, Wife of Veteran A discovered the package also included medical records for Veteran B. The wife is still in possession of the records as of now but has informed ROI that she will bring records back to facility tomorrow. Update: 01/25/11:Veteran B will receive a letter offering credit protection services since his full SSN and DOB was exposed.

Outcome:

Supervisor conducted a counseling in terms ensuring in the future that every single page (copied) is reviewed prior to release; the ROI Clerk apologized for the error. The employee was also informed of the impact of the error, e.g. costs to the VA for one year of credit counseling, reporting of breach to Quad, VACO, PR, etc.
