Issue:
On 04/30/15, the privacy Officer (PO) received report of medication and medication and/or discharge paperwork given to Veteran B for Veteran A. Veteran B reported it to the nurse, who self-reported and contacted Veteran A. No information regarding document recover. Additional notification and investigation, to ensue.
Outcome:
05/04/15: The Incident Resolution Service Team has determined that Veteran A will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
