Issue:
Veteran A came to the Pharmacy window this afternoon and told Employee B he had received someone elses paperwork along with his own. Employee B reviewed everything in the bag that Veteran A presented. It included Veteran A's medications and his paperwork which Employee B reviewed for correctness (Veteran A's prescriptions and paperwork were from 08/09/12). However, there was also paperwork from Veteran C which included part of a page describing the drug information for his prescription and Veteran C's medication summary page, dated 08/08/12. Update: 08/13/12:Veteran C will receive a HIPAA letter of notification.
Outcome:
Pharmacy Supervisor will re-train and educate all staff on the importance of reviewing each and every paper for accurate patient information prior to providing medications to the patients.
