Issue:
A transition package for one Veteran faxed from the military VA intake site was faxed to the Operation Iraqi Freedom/Operation Enduring Freedom (OIF/OEF) program coordinator. The package was put in a folder and given to the Veteran to start their transition from the DOD to VA health care. While in the waiting room the Veterans wife discovered an email with five other patients full names, last four numbers of the SSN and a summary of medical problems in the Veterans packet. The information was brought to the attention of a VHA employee who took possession of the other Veterans' information email.
Outcome:
11/13/14: The Incident Resolution Service Team has determined that the five Veterans whose information was disclosed will be sent a notification letter.
