Issue:

It was reported to the pharmacy by Veteran A that he did not receive his Fentanyl medication. After further investigation, it was discovered the prescription was delivered to Veteran B, in error. Both of these Veterans are prescribed Fentanyl medication/same strength/same dose and same directions for use. Veteran B was notified and confirmed receipt of Veteran As prescription, stating that he did not notice it was not his. Veteran B is returning the medication to this facility pharmacy. Veteran A's name and medication information was compromised.

Outcome:

06/25/14: The Incident Resolution Service Team determined that Veteran A will receive a HIPAA letter of notification.
