Issue:

01/10/11 - The Roanoke Regional Office mailed 7 Counseling Evaluation and Rehabilitation (CER) files to the Richmond outbased office, located at McGuire VAMC. 01/11/11 - The CER files were received at the loading dock and the 0925 was signed by an employee named: Richmond VR&E staff made numerous inquiries and requests for assistance from the McGuire VAMC staff to include contacting the warehouse and mailroom with no luck in tracking down the missing files. The Hospital's records department and the Office of Compensation and Pension were also contacted with no results.We notified Roanoke Admin and the ISO re: our concerns. The ISO contacted the ISO at Richmond VAMC. Again, despite repeated attempts to locate the CER files, they remain missing. Update: 03/03/11:Seven (7) Veterans will receive a letter offering credit protection services.

Outcome:

Procedures have been established between the Director's Office and ORC for documents that are sent to this facility.
