Issue:
Employee A's competency folder including his recent proficiency report and performance award information was misplaced by their supervisor. The employee's supervisor was taking the folder from one clinic to another and did not properly safeguard it. It is believed that it was misplaced in a parking lot and has not been recovered. The performance award contained the employee's full name, full social security number and award amount.
Outcome:
03/06/15: The Incident Resolution Service Team has determined that employee A will be sent a letter offering credit protection services.
