10/20/14: The Incident Resolution Service Team has determined that there was a potential policy violation. While there was the potential for an unauthorized access/disclosure of data, it has been determined that the only persons who could have seen the Veteran's data were VA staff, USPS staff performing their official duties, and the Veteran.
