Issue:

Today, 08/07/13, Pharmacy supervisor reports that Veteran B and Veteran A called him to report that Veteran B received two of Veteran As medications. Veteran B was not expecting medications so he did not open medications packages delivered to him and called the Pharmacy. Veteran B did not receive medications as expected, went to his Community based Outpatient Clinic (CBOC) appointment where he received information causing him to call Pharmacy. Pharmacy identified an address and phone change made in another department on 06/21/13. Prior incorrect address changes noted in privacy events, same staff, were provided by VBA, overriding facility address. Pharmacy corrected Veteran As address and sent mailer to retrieve medications. Medications are not technically controlled substances. Veteran A was supplied with newly refilled medications. Recovery of medications in progress. Data exposure to be validated upon receipt. Additional notification and investigation, pending.

Outcome:

08/08/13: Veteran A will receive a HIPAA letter of notification.
