Issue:

VA Employee A delivered VA form for computer access and rules of behavior to VA employee B unsecured. This VA form contained information regarding VA employee C (name, SSN, address, phone, email, date of birth, gender). VA Employee B was not the intended recipient. The PO Spoke with VA Employee A who delivered the form who states the form was delivered unsecured. Presumably, VA employee B delivered the form to VA Employee/Supervisor D but have not confirmed with Employee D that she received the form. This event is still under investigation. Update: 08/26/11:The investigation revealed that Employee C's data was left unsecured. Employee C will be sent a letter offering credit protection services.
