Mentioned in a privacy incident report created by the U.S. Department of Veterans Affairs on March 3, 2015. Also cited in 64 other reports.

Report ID: PSETS0000116141, U.S. Department of Veterans Affairs

Reported Entity: NORTH FL/SOUTH GA HCS-GAINESVILLE - 573

Issue:

A VHA employee alleges a co-worker inappropriately accessed her medical record. A Sensitive Patient Access Report (SPAR) indicates the co-worker accessed her medical record on two occasions. The VHA employee is a non-Veteran who has never been seen at that clinic so there is no reason co-worker should have accessed her record. Privacy will investigate.

Outcome:

03/03/15: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being accessed without a need to know.
