Issue:

Veteran A received a medication summary routing slip belonging to Veteran B. Veteran A returned the document within 30 minutes. Veteran A states he was given the document by a physician in the clinic. Veteran A returned the document to a medical support assistant who in turn provided the document and facts of the incident to the Privacy Officer (PO) on 05/07/12. Update: 05/07/12:Veteran B will be sent a letter offering credit protection services, as his full SSN and health information including diagnosis, medications, and date of birth were disclosed to Veteran A.NOTE: There were a total of 30 Mis-Handling incidents this reporting period. Because of repetition, the other 29 are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.

Outcome:

The PO provided education to employee. All other actions wil be handled by Human Resources (HR) and the employee's Service.
