Veteran A called to report that he received a package from the Tomah VA Prosthetics Department with his full SSN and date of birth (DoB) on the mailing label. The SSN and DoB had been crossed out with a black marker but one could still see SSN and DoB. Update: 04/18/13:Veteran A will receive a letter offering credit protection services.05/08/13:This was determined to be HITECH reportable by VHA Privacy Office.
