Issue:

On 10/31/12, Veteran A presented to Release of Information (ROI), Nashville Campus, and obtained two copies of her medical records. After arriving home, Veteran A put the documents aside and did not review the information until 11/07/12. At this time, Veteran A noticed she had also received medical information (89 pages) belonging to Veteran B.Today, 11/09/12, Veteran A presented to Patient Advocate Office, Nashville Campus, to report the incident. Veteran A returned Veteran Bs documents to the Patient Advocate, Nashville Campus, who reported the incident to the Privacy Officer (PO), Nashville Campus. PO will obtain Veteran Bs documents from the Patient Advocate, Nashville Campus, for investigative purposes. Veteran Bs personal identifiable information (PII) includes: full name, full SSN, DOB, address, medication lists, and progress notes. Update: 11/09/12:Veteran B will receive a letter offering credit protection services.

Outcome:

11/14/12 - Fist incident regarding this nurse. Employee counseled. CM letter mailed.
