Issue:
Based on interview and record review, the hospital failed to safeguard Patient 1's protected health information (PHI) from unauthorized persons.Findings:The information received from the hospital Privacy Officer, on 2/5/13, indicated Laboratory Staff 1 entered the wrong fax number while attempting to fax laboratory results to Patient A's physician. The fax number was immediately changed. Staff was re-educated to check the fax number before faxing information.
