Issue:
Veteran A had requested and was provided medical records from the release of information office. Veteran A however sent back several pages of the documents that were released which contained correct name, SSN and date of birth, but the notes pertained to Veteran B which has the same first and last name but has a different middle name. It has been verified that the incorrect medical information was entered into Veteran A's record. The record is in the process of being corrected by the Privacy Officer and health information staff. Update: 05/16/12:Veteran B will be sent a notification letter.
Outcome:
The records have been corrected and notification has been completed. This error will be addressed to the COS office to advise that proper medical documentation is very important.
