Issue:

Veteran A contacted the Health Resource Center (HRC) to report that he had received a letter addressed to him, which contained Veteran B's information in the form of a bill statement and bill profile (full social security number, address, account balances, and transactions). Veteran A has returned the statement to the HRC and provided the facility with a scanned copy. Update: 06/13/12:Veteran B will be sent a letter offering credit protection services.

Outcome:

Documents returned and destroyed. Section Chief alerted in order to made the necessary corrections to the record, and prevent any future disclosure errors.
