Issue:

Veteran A called the Dorn VA medical Center Pharmacy to inform them that she received a package addressed to her from the Dorn VA. Upon opening the package, she realized the package contained medication intended for another Veteran B. Veteran A said that she had disposed of the medication and paperwork, and thought she would call the pharmacy to let us know about the incident. The information disclosed: Veteran B full name, address, name of provider, and the Rx name and instructions. Update: 02/11/11:Veteran B will be sent a letter of notification.

Outcome:

Corrective Action: Employees have been educated on the need to safeguard patient information.
