Issue:

The Laboratory Supervisor was notified by Employee A that Patient A submitted a stool sample container with the full name, full social security number and date of birth of Patient B. Laboratory Supervisor reported that Employee B provided the stool sample container to Patient A with the wrong specimen label of Patient B. The husband of Patient A contacted the laboratory and notified Employee B of this error. The husband of Patient A was told to put the correct patient information of Patient A on the specimen sample. When specimen was returned, it was provided to the Laboratory Supervisor who destroyed the sample per laboratory procedures.

Outcome:

06/22/14: The Incident Resolution Service Team determined that Patient B will receive a letter offering credit protection services.
