Issue:
The Edgewater Nursing Home staff brought Veteran A to the facility for an appointment. During the course of transporting the Veteran, they dropped documents containing the Veteran's name, full SSN and diagnosis. The Privacy Officer (PO) called the Nursing Home and they verified the documents belong to them. The documents were found by a Quality Management (QM) Nurse who then contacted PO. Update: 10/26/11:The QM Nurse found the documents in the hallway near the first floor elevators. They were found at 2:30 PM and the patient left the clinic at around 2:00 pm. There is a moderate amount of foot traffic in that area. There are no cameras in the area.10/27/11:VA has a contract with the nursing home. We will provide credit protection services since the documents were unattended for 30 minutes.
Outcome:
Staff has been re-educated.
