Issue:

Based on interview and record review, the facility failed to ensure that Patient 1's medical record was kept confidential from anyone not authorized access.Findings:An interview with the hospital's Privacy Officer (PO) was conducted on 5/3/12 at 9:00 A.M. The PO stated that Patient 1's laboratory order was mistakenly given to Patient 2. According to the PO, Patient 2 went to the clinic to get her blood work done, and that is when the clinic employee noticed that the laboratory order that Patient 2 had in her possession, belonged to Patient 1. The laboratory order contained Patient 1's name, date of birth, diagnosis, name of insurance, and the ordered laboratory test.
