Issue:

The Research Compliance Officer requested report on a Research Study provider who failed to complete the HIPAA authorization correctly. The PI willingly reported an additional study having the same issues. Information included date of birth, last 4 SSN, admission and discharge dates, emergency room date and death date was entered on subjects into the Affiliate University system without patients HIPAA authorization. An amendment was approved by the IRB which added that data is stored at both VA and affiliate. The subjects were not re-consented with the revised authorization. The study is completed.

Outcome:

06/26/13: Thirty-Seven (37) Veterans will be sent a notification letter.
