Issue:

Veteran A received a prescription package by UPS which contained a narcotic that was intended for Veteran B. Both Veterans have the same first and last name. The Pharmacist mislabeled the package with the Veteran A's name and address. Veteran A's wife contacted the narcotic Pharmacist. She signed for the package on 4/7/2011, but did not notice that the middle initial printed on the label of the bottle was different. The Pharmacist apologized for the error, and the wife was very appreciative. According to the Veterans wife, no medication was consumed. The Pharmacist contacted Veteran B and explained the situation. The correct medication was forwarded by UPS to Veteran B. Veteran B was very appreciative and thanked the Pharmacist for a job well done. The medication was returned to the Dorn VA Pharmacy on 4/21/2011. This incident was documented in CPRS on 4/7/2011, and not reported to the PO. This incident is being reported to the Patient Safety Team. Update: 05/10/11:Veteran B will be sent a notification letter, due to his name and medication being disclosed.

Outcome:

Corrective Action: Staff is being re-educated on the need to safeguard patient information.
