Issue:

Three prescriptions for controlled substances for two Veterans were sent to the Castle Point campus via UPS. The package was signed for by a VA employee but the prescriptions were never sent to pharmacy. The prescriptions are now missing. The prescriptions contained name, full social security number, address, date of birth for each Veteran. Update: 01/11/13:Both Veterans will be sent a letter offering credit protection services.

Outcome:

Deliveries that do not belong to the Warehouse will not be accepted and/or signed for. Deliveries marked for the Pharmacy will not be accepted by the Warehouse staff. The tracking log currently in use, will be reviewed to implement a clear process of tracking and final distribution of all deliveries. Warehouse staff will be present when UPS/Fed EX deliveries are being unloaded. Staffing issues will be addressed by recruiting additional staff.
