Based on interview and record review, the facility failed to safeguard Patient 1's Protected Health Information (PHI) from unauthorized persons which resulted in a breach of her PHI.Based on interview and record review, the facility failed to safeguard Patient 1's Protected Health Information (PHI) from unauthorized persons which resulted in a breach of his PHI.Findings:During a review of the facility Privacy Complaint Follow-up Report, dated 5/17/13, it indicated Patient 1 had been seen in the Emergency Room on 5/14/13. Upon his discharge, Registered Nurse A (RN A) faxed his discharge summary to the wrong physician. The facility investigated the incident and determined that the wrong physician was chosen from the drop down menu. RN A was re-educated and instructed to check the physician's name twice prior to faxing the information.
