Issue:
Veteran A received a package with medication inside of it. Upon opening it, Veteran A noticed it was Veteran B's medication. Veteran A notified the Patient Advocate. Veteran A was asked to return the package via UPS to the Pharmacy Supervisor.
Outcome:
12/03/14: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
