Issue:

While in the Canteen, the Privacy Officer (PO) noticed a sheet of paper on top of a chair located by one of the Vendors. While reviewing the document, the PO noticed it was full of patient information. The Vendor stated, oh yea, I found that yesterday and put it up there. It has one patient's full name, last four digits of the SSN, vital signs, history, diagnosis, type of surgery, medications and labs, all handwritten on the back of a Anesthesia Preoperative Evaluation form. The patient had a surgery yesterday.

Outcome:

12/10/14: The Incident Resolution Service Team has determined that Patient A will be sent a notification letter.
