Issue:

Based on staff interview and record review, the facility failed to maintain the privacy and confidentiality for seventeen patients (Patient 1 through 17), when a pharmacy was burglarized and personal receipts were stolen.Findings:An interview was conducted on September 13, 2012 at 10:30 AM, with the facility's Privacy Officer (PO). The PO stated that on August 30, 2012, she became aware of a breach incident. The PO stated that on August 29, 2012, a pharmacy that the facility used was burglarized early in the morning. The PO stated that receipts were taken for 17 patients that had used the pharmacy to fill prescriptions.A review on September 13, 2012 revealed the following personal health information (PHI) that was taken included:Patient nameAddressDate of birthTelephone numberPrescribed medicationThe PO further stated that all 17 patients were notified of the breach incident.
