Issue:

Employee left a folder containing PHI/PII in an unsecure area. Privacy Officer (PO) investigating and awaiting more detailed information. Update: 11/13/12:One Veteran will be sent a letter offering credit protection services due to full name and full SSN being exposed. The Veteran's proxy, whose full name and address were also in the folder, will receive a general notification letter.

Outcome:

The employee responsible has been counseled by their supervisor and is assigned to re-complete VA Privacy and Security Awareness Training. The employee is not authorized to remove any documents from the area unless the documents are in a secure tote. Notification letter sent to proxy. Credit monitoring letter uploaded and cannot be mailed at this time due to no known address. PO and clinical team monitoring for patient location so that the letter may be delivered as soon as possible.
