Issue:

A physician reported that during a medical visit with Veteran A he left the room for a short period of time. When he returned Veteran A confronted him about leaving a document in the room that contained Veteran B's name, partial SSN and vital signs. He told the provider he had violated HIPAA. Veteran A had taken a photo of the sheet of paper using his personal cell phone. The physician asked him to delete the photo and he refused saying he was going to the Houston VA to report the physician.

Outcome:

03/18/15: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
