Issue:

Veteran A presented to the Topeka ROI in-take Office May 16-12 and reported hed received Veteran Bs medical information which had been incorrectly scanned into his medical record. Veteran A had appropriately/legally requested the information. The ROI in-take specialist tried to retrieve the information from Veteran A; however the Veteran stated hed have to speak to his attorney. Update: 05/18/12:Veteran B will be sent a notification letter.

Outcome:

Veteran A - no longer has the incorrect information in VistA Imaging & notification will be sent to him as well as the second individual to clarify this unfortunate circumstance is resolved.Re-education was provided to scanning clerks to always double check when printing & releasing scanned items that it is indeed the correct patient information -in case this situation is found - it can be reported & corrected. A lot of information is scanned in our veterans' records (not only by Medical Record's scanners) and any safeguard to decrease the risk is our responsibility.Will provide the individuals with notification letters ASAP. Letters sent & Redacted notification letter uploaded and HIPAA Notification Letter uploaded.
