Issue:
A Quality Management Specialist discovered a sheet of paper with three patients names, last four digits of the SSNs, and test results from anticoagulation clinic. The sheet was laying face-up on a projector cart in a conference room and was discovered by the Specialist when she entered the room for a meeting. The room had been in use by other groups that day. The Specialist secured the sheet and is sending it to the Privacy Officer for follow-on investigation and action. Further updating upon receipt of the document. Update: 02/16/12:According to the Privacy Officer (PO) it is believed it was left in the room by the preceding group, which was a "meter(?)" class. The PO needs to ascertain who all was in attendance. There's a probability that the names on the list were the attendees (patients), since that was classified as a patient education course. The PO is still looking into the security of the conference room.02/17/12:The area was accessible to the general public. Therefore three (3) Veterans will receive a nitrification letter.
Outcome:
Employee was verbally counseled regarding necessity to properly safeguard patient data and reminded to cross-check room before leaving.
