Issue:

A lab technician found a doctor's order for mammography that had been faxed from a non VA health care facility on the table outside her office where patients sign in for laboratory testing. The order contained the patient's name, date of birth, requested testing, and reason for exam. The lab technician contacted her supervisor and the alternate Privacy Officer. Update: 06/08/12:The patient will receive a letter offering credit protection services.
