Issue:

The CMOP mailed medication to two different Veterans that were returned to the hospital's pharmacy due to bad addresses. The Pharmacy Technician called the Veterans to obtain their correct addresses. When medications were sent back out, they were both placed in the same envelope. The Veteran who received the envelope open both packages before realizing one was not his. The Veteran's daughter returned the medication received in error. Veteran A and his daughter were privy to Veteran B's name, address, and medication. Update: 03/27/12:Veteran B will be sent a notification letter due to Protected Health Information (PHI) being disclosed.

Outcome:

Supervisor spoke with employee regarding importance of assuring medications are sent to the correct address and the right Veteran.
