Issue:

Veteran A requested and was provided an imaging CD to hand carry to his civilian provider. Veteran A reports that at his appointment with his civilian provider, he was told the data on the CD was not his. Veteran A hand carried the imaging CD back to VHA and reported this issue to VHA employees. Update: 02/03/12:Veteran B will be sent a letter offering credit protection services.

Outcome:

Service Line Mgt has re-educated the employee about handling PII. Action has been taken to remove the employee from the designated position.
