Issue:
A request for information was received with an authorization form attached. However, the authorization form specifically states that the patient DID NOT wish to have information regarding drug and alcohol abuse, mental illness, AIDS or HIV, STDs, tuberculosis or genetics released. The request was for all medical records. All medical records were released, including drug/alcohol and mental illness related information. Update: 11/09/12;The patient will receive a HIPAA letter of notification.
