Issue:
A Veteran contacted our LCSW stating that he received paperwork regarding an appointment of his along with multiple documents on another veteran (Informational Prescription Profile). This information included diagnosis, prescriptive drugs, name, DOB, Address, Phone number, as well as other medical information. This Veteran was asked if he would mind returning the paperwork to the VA's possession and he returned all papers that he received. The LCSW, in turn, contacted the Privacy Officer and returned all patient paperwork. Total of 8 pages. Update: 09/10/12:One Veteran will be sent a letter offering credit protection services due to full name, date of birth, and address being disclosed.
Outcome:
Unable to determine who mailed incorrect information to Veteran. All medical information was retrieved.
