Issue:
Veteran A reported that he was given paperwork after an appointment and co-mingled with his paperwork was an Appointment Schedule that listed his name and six other Veterans appointment times, full name, full SSN, phone number, service connect percentage, what the appointment was for, and means test data. Veteran A returned document to the Privacy Officer. Update: 02/02/12:Six (6) Veterans will be sent letters offering credit protection services.
Outcome:
Three nurses printed out the appointment schedule, one nurse gave her schedule to the Provider; another nurse had already printed out one for the Provider and placed it on the nurses counter. A nurse accidently picked it up while collecting patient education material and plan of care. All staff reminded about privacy policies. Changes in procedures such as only one schedule will be printed for all nurses; Provider will receive a copy personally, annotated with the Provider name, no PHI/PII left on the nurses station counter. Credit monitoring letters mailed February 2, 2012 and uploaded
