Issue:

Medical reports belonging to Veteran A was inappropriately released to Veteran B. The documents were received in the mail from Veteran B today with a note that reports were sent to her in error. Veteran A's documents were included in with the documents that were requested by Veteran B.Documents consisted of seven documents (radiology reports and EKG) including complete social security number, date of birth and health information.Per research of the Release of Information (ROI) Disclosure software, medical reports were inadvertently mailed to Veteran B on 07/15/12 by a Release of Information Office staff member. These documents were returned via the mail on 7/19/12 by Veteran B. Update: 07/19/12:Veteran A will be sent a letter offering credit protection services.

Outcome:

Staff is being re-educated on the need to safeguard patient information.
