Issue:

A VA dermatologist reported to the Privacy Officer on September 22nd that on September 10, 2014, at the end of her tour of duty she discovered that a sony digital camera the Service has been using to take pictures for patient care was missing from the work area. She stated the incident was reported to the VA Police. She and her co-workers have been searching for the camera and have not been able to find it. The camera contains photographs of two Veterans with labels which display their full name, SSN and date of birth.

Outcome:

09/22/14: The Incident Resolution Service Team has determined that Patient A and B will be sent a letter offering credit protection services.
