Issue:

The Privacy Officer (PO) received call from Employee reporting that a Veteran brought him a green log book with 13 names and SSNs to turn in. The log book was in a box located on top of a shredder bin with empty boxes and old pamphlets. The Veteran carried the book directly to the employee upon finding the names in it, (names were located inside the log book on the third page). PO called the section manager who reported that they placed the book there and did thumb through it and did not see this list of names on the page. PO and manager confirmed that there was no other information located in the box and no missing pages in the log book. Update: 06/26/12:Thirteen (13) Veterans will be sent letters offering credit protection services due to full name and SSN being exposed.

Outcome:

Employee produced training certificate, supervisor notified. Green log book destroyed properly. Notification letters mailed today.
