Issue:
UPS delivered a National Practitioner Data Bank Review package to a VA employee's home address. The package originated from the Quality Management (QM) Service. The address label had the correct recipient's name, but VA employee's home address. Two delivery attempts were made by UPS, with the second and final attempt delivered to the apartment complex office, with a note to the VA employee that delivery was made. The employee subsequently opened the package and discovered it was not for him, whereupon he re-packaged the envelope and sent through interoffice mail to the Privacy Officer. The Employee works at a CBOC and is 60 miles distant. We are in the process of determining how the employee's address (with same last name as intended recipient) was paired with the addressee's name. Update: 10/25/11:The Veteran whose PHI/PII was disclosed will be sent a letter offering credit protection services.
Outcome:
The Risk Manager will confirm and re-verify the most current known address for all intended recipients of tort claim notifications effective immediately. Credit monitoring letter provided to Veteran.
