Issue:
The ISO was informed that a Veteran received a phone call at their residence on 05/02/14, from a VA Staff person who asked for the Veteran by name. The Veterans wife answered the call and stated that the Veteran was at work, at the Phoenix VAMC at that time. The caller stated that the Veterans lab results had come back and the caller proceeded to share PHI with the Veterans wife. The Veteran stated that at no time did the caller ask who they were talking to and the Veterans wife never indicated who she was. The Veteran believes it was someone from their assigned clinic calling and is concerned about the information being shared.
Outcome:
05/05/14: The Incident Resolution Team has determined that Veteran A will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
