Issue:

A Veteran received his non-VA care prescription from the VA Clinical Pharmacist and noted it contained prescription information pertaining to another Veteran. He reported the incident to the VA Outpatient Pharmacy. Upon review of the incident, Outpatient Pharmacy requested the mismailed prescription information to be forwarded to the VA. The personally identifiable information (PII) pertaining to the affected Veteran believed to be inappropriately exposed was the full name and date of birth.

Outcome:

12/31/14: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
