Issue:

Patient believes a Lorain employee inappropriately accessed his record.

Outcome:

03/04/15: PO update -- Pull report and called patient. Awaiting call back. The report does not reflect the suspected person as accessing the record. 03/05/15: PO update -- Spoke to the spouse (caregiver) today. She relayed the name of the employee. The employee in question did in fact access the record on 2-25-15 (CPRS). My quick review reveals that the patient was not seen at the Clinic on that day. The spouse stated they became suspicious when the employee sent a face book message that outlined some medical information. The patient states he does not discuss his medical issues with people. The two knew each other from the service and the employee just happens to be employed at the VA. 03/06/15: PO update -- Interview scheduled for Monday, March 9. Supervisor and patient's treatment team notified. The treatment team is concerned as the patient does not want to come to the VA anymore. 03/13/15: PO update -- After a fact finding review with the employee. The employee admitted he had no reason to access the record. Changing to incident. There is no evidence that he printed information or disclosed information, rather he was snooping into a past military friends record. 03/16/15: The Incident Resolution Service Team has determined that Veteran A will be sent a notification letter.
