Issue:

Today, 5/30/2014, Veteran reports privacy incident to PO. He returned medical records of another Veteran, received from this facility by mail in late 2013. Veteran had requested his own medical records but received both sets of records. Veteran does not live in this city, receiving routine care at a rural CBOC. Until his visit at facility today, he has stored other medical records securely in a locked file cabinet; were not shown to anyone else and confirmed no copies were made. Reassured him that his qualitative chart audit would be conducted as part of the investigation along with information protection for the other Veteran. Initial review of medical record disclosures indicates that records were processed for both Veterans who had the same last common name with same last 4 of SSN. Additional investigation and notification to supervisors, to ensue.

Outcome:

06/02/14: The Incident Resolution Team has determined that Veteran B will be sent a letter offering credit protection services.
