Issue:
On 4/24/12, the Privacy Officer received a telephone call from Veteran A. Veteran A reported that he requested copies of his medical records from the Release of Information Office (ROI). His request was processed by the Alvin C. York Campus ROI office on 4/10/12 and mailed out on 4/19/12. Veteran A stated that when he opened the envelope, he immediately noticed he also received medical records of some other Veterans. He immediately put the medical records of the other Veterans back in the envelope and sealed them. He was unsure of how many other Veterans records he received, but thought there were two others. He agreed to take the sealed envelope to the VA Chattanooga Outpatient Clinic, who will in-turn forward them to the Nashville Privacy Officer. Update: 05/09/12:One Veteran will be sent a letter offering credit protection services.
Outcome:
CM Letter to be mailed this date, 5/11/12. Release of Information Supervisor provided training to all ROI Staff concerning the important of performing quality checks on outgoing requests. PO also recommended the Supervisor make routine Quality Checks as well and document.
