Today, 03/26/15, the Privacy Officer (PO) received a privacy incident report from a Supervisor on behalf of Veteran A. Veteran As daughter reports that Veteran A received Veteran Bs medical records when his records were received this week. The disclosure page count is over a ream of paper, consisting of medical records for the past five years including sensitive, 38 USC 7332, protected information. Recovery efforts and reprocessing of records are in process. Additional notification and investigation, to ensue.
Outcome:
03/27/15: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
